Physician Oversight and Medication Management
A hospice physician will be assigned to the patient’s case and must actively participate in all cases. They will manage oversight of all aspects of a patient’s care. This includes hospice eligibility, medication management, care plan development, crisis management, quality improvement and advocacy while working toward the common goals of pain and symptom management. This may include an occasional in-home visit from a nurse practitioner when needed.
Nursing Support & Services
The RN Case Manager acts as the patient’s first point of contact and advocates on the common goals decided upon by the hospice team and patient/family. The RN Case Manager will develop a care plan for all involved hospice team members to follow, will coordinate with all parties involved in the patient’s care, perform medication management, educate on the various aspects of care ranging from medication, care or use of equipment. The RN Case Manager will continuously monitor and manage symptoms and issues as they arise. Our LPNs play a significant role in supporting the patient and team. From assisting with making routine visits or sitting bedside with our patients that have a condition or symptom that need monitoring and a higher level of management. Additionally, the LPNs will help with continual education of our patients and families while carrying out the plan of care.
Home Health Aide Care
The hospice aide plays a vital role in ensuring patient and family comfort through assisting with activities of daily living including but not limited to bathing, dressing, grooming and restroom activities. They help to ensure the patient and family are well care for and their needs are met. Our home health aides also provide companionship and emotional support to our patients.
Social Worker Reassurance
Social Workers often describe their jobs as an advocacy-based calling, for those that are sick, injured, displaced, and/or in need of support, or resources. Our social workers are essential in providing emotional and practical support for our patients and their families. This may mean finding a facility for a respite stay, helping to comfort and reassure, as well as strong advocating for the various treatment options and plans our patients and their families may be presented with. Our social workers can also assist with any mental health needs as well.
Chaplain Support
Spiritual and emotional support is often necessary for our patients, and that’s where our Chaplains come into play. These team members help our patients, and their families navigate the non-medical challenges, address spiritual needs, can be a listening ear or provide some companionship. Our chaplains are non-denominational, and can be a resource to the faith based community if so desired.
Integrated Therapies
We often discover our patients find joy, comfort and thrive just a little bit more in the extras. Ranging from massage to music therapy, or perhaps bringing in a pet to soothe the soul. We believe in and will accommodate as many requests as possible to ensure the needs and wishes of the patient are met.
