Levels of Care
Routine Care
The vast majority of our patients will be under a routine level of care 95% of the time. Routine care visits truly depend on the needs of each patient. It includes nursing and aide visits several times a week, with supplementation from non-medical visits from the social worker and chaplain a few times a month or more based on patient need.
Respite
Here and there, caregivers for our home patients may need a break for a variety of reasons. Most insurance plans will cover a short respite stay at a Medicare enrolled nursing facility. This five day stay helps combat caregiver burnout, supporting the patient and caregiver.
Continuous Care
Continuous care is symptom management based short term level of care, where we have staff at bedside to help manage symptoms. In order to qualify for this level of care, there are very specific requirements a patient must meet, so this isn’t for every patient. We will always advocate for our patients and the correct level of care.
General Inpatient
General Inpatient or GIP is our highest level of care, and requires significant intervention. This level of care is typically performed in a hospice or skilled nursing facility, heavy involvement from hospice team and the facility staff. Very few patients will qualify for this level of care during their hospice stay.
